LECs choosing to use the SMS/800 IntraLATA Carrier Management feature to manage use of the 0110 Carrier Identification Code (CIC) for specific Operating Company Numbers (OCNs) owned by their company must submit the SMS-47 Operating Company Numbers (OCN) form, located under Forms, to the SMS/800 Help Desk.

By completing and submitting the SMS-47 Operating Company Numbers (OCN) form the LEC certifies the OCNs listed on the form belong to their company. Upon receipt of this form, the SMS/800 Help Desk populates the OCN screen in the SMS/800 with the OCNs listed on the form.

LECs choosing to allow or block use of the 0110 CIC for specific POTs numbers or 1K blocks owned by their company, must submit the SMS-110 RAC/RDC Creation for 0110 Use form, located under Forms, to the SMS/800 Help Desk.

By completing and submitting the SMS-110 RAC/RDC Creation for 0110 Use form the LEC certifies the POTs numbers or 1K blocks listed on the form belong to their company.  Upon the receipt of this form, the SMS/800 Help Desk populates the RAC or RDC screen in the SMS/800 with the POTs numbers or 1K blocks listed on the form.

The SMS-47 Operating Company Numbers (OCN) and SMS-110 RAC/RDC Creation for 0110 Use forms can be faxed or mailed to the SMS/800 Help Desk via the contact information on the bottom of the forms.
